چکیده

Introduction Transcatheter aortic valve implantation (TAVI) represents a new treatment option for stenosis. This study aimed to evaluate the cost utility of TAVI, compared with clinical treatment, in patients inoperable severe stenosis from perspective Brazilian public health system. Methods A Markov model monthly cycles and five percent annual discount rate was constructed. five-year time horizon chosen, minimize uncertainties inherent data extrapolations, based on only randomized head-to-head trial, Placement AoRTic TraNscathetER Valve Trial (PARTNER B). All costs were obtained Brazil's official healthcare data. Utilities 0.6 (range 0.56–0.63) TAVI 0.71 0.69–0.72) studies that used EuroQol-5D instrument. TAVI's measures penalized by 25 first month, estimate procedure's impact quality life provided National Institute Health Care Excellence United Kingdom. Lastly, deterministic probabilistic sensitivity analyses robustness results. Results The incremental cost-effectiveness ratio USD35,880 per quality-adjusted life-year (QALY), result mainly sensitive univariate analysis. In analysis, all values above reference willingness-to-pay threshold three times capita gross domestic product (USD18,042 QALY). Conclusions conclusion, even though there is no established Brazil, may represent an obstacle its incorporation into

Transcatheter aortic-valve implantation for aortic stenosis.

BACKGROUND Short-term results are presented from our first year of experience with transcatheter aortic-valve implantation [corrected] (TAVI) at the University Hospital of North Norway. MATERIALS AND METHODS TAVI was used to treat 25 patients with aortic stenosis [corrected] from September 2008 to August 2009. 25 patients operated with conventional aortic valve replacement (AVR) served as con...

Transcatheter aortic valve implantation for patients with severe bicuspid aortic valve stenosis.

BACKGROUND Bicuspid aortic valve (BAV) is regarded as a relative contraindication to transcatheter aortic valve implantation attributable to the risk of uneven expansion of the bioprosthesis. The purpose of this study was to evaluate the efficacy and safety of transcatheter aortic valve implantation in patients with BAV. METHODS AND RESULTS Of 470 patients included in our prospective transcat...

Transcatheter aortic valve implantation in patients with severe aortic stenosis and small aortic annulus.

OBJECTIVES Valve hemodynamics and clinical outcomes among patients with a small aortic annulus who underwent transcatheter aortic valve implantation (TAVI) were examined. BACKGROUND The presence of a small aortic annulus may complicate the surgical management of patients with severe aortic stenosis (AS). TAVI is an alternative to aortic valve replacement (AVR) in high-risk patients, but few d...
